Home Visits
Exceptional care in the comfort of your own home. We bring the clinic to you! From infancy until the last days, Sarah will examine, diagnosis, treat, and provide ongoing care for your whole family. We do it all- acute and chronic health visits, sutures, simple procedures, sports physicals, and referrals for injuries, specialty consultation, and surgery.
Virtual Visits
Telemedicine appointments are available conveniently from your smart phone over a HIPAA compliant connection. Video visits are great for medication refills, simple acute concerns, follow up visits, and consultations. Same day appointments available!
After Hours Visits
We know that your medical needs don't stop when office hours are over. Extended hours are available for both telemedicine and home visits. If something occurs outside of published hours- let us know and Sarah will do her best to get you taken care of.
Wellness Support & Weight Loss
Sarah will support you in building a healthier you. We work together to come up with a personalized wellness plan and get you connected to the services you need.
Hormone Replacement Therapy
Feel your best with customized hormone optimization services. Packages include evaluation, labs, individualized treatment plan, and medications.

An initial appointment is required to establish goals, order lab work, and review health history. We utilize several different weight loss and wellness protocols to help you meet your goals and pricing varies based on the chosen treatment plan to best suit your individual needs.

Payment
Regular hours are Monday-Friday 9am-5pm. After hours, weekend, and urgent visits are available for an additional cost.
At this time, all services are cash-pay only and must be paid in full directly to the provider at the time services are rendered.
A superbill can be provided upon request for you to provide to your insurer for potential reimbursement of out-of-network services. While we can offer no expectation or guarantee of reimbursement, our patients report anywhere from zero to 80% being covered by their insurance providers.
Payments can be made via
Cash, Check, Square, or Venmo
You may use the credit card connected to your Health Savings Account (HSA) or Flexible Spending Account (FSA) for out-of-network healthcare services.
